Watch for sustained overtime, declining velocity over consecutive sprints, and increasing sick days. These are leading indicators that utilization has crossed from productive into destructive.
Finally, track client satisfaction at project milestones, not just at completion. A client who is unhappy at the midpoint and happy at delivery had a stressful experience. A client who is satisfied at every checkpoint had a good one, even if the final deliverable is identical.

Scope creep is real and destructive, but not every scope change is creep. Early-stage changes often represent legitimate discovery, where the team learned something that changes what should be built. Late-stage changes with no new information are creep. Your framework should distinguish between the two and handle them differently. Rejecting all scope changes creates rigid projects. Accepting all of them creates projects that never finish.
